Role Description

The Patient Care Manager will lead care coordination efforts, support patient engagement, and ensure the delivery of high-quality, patient-centered services across virtual care platforms. This role serves as a key liaison between patients, healthcare providers, and internal teams, ensuring seamless communication and effective care management. This position is ideal for a healthcare professional with strong operational expertise, leadership experience, and a commitment to improving patient experiences through efficient and organized care delivery.

  • Oversee daily patient care coordination and remote support services
  • Supervise, mentor, and support patient care coordinators and support staff
  • Ensure timely patient follow-ups, care updates, and service assistance

Qualifications
