About the Role

As the RN Lead, you will play a crucial dual-role as both a clinical expert and an operational leader within the Integrated Care team. You will manage and mentor our Care Coordination staff, serve as the central point of clinical support, and help define and scale the entire nursing function at Maximus. Working directly with the Medical Director and Practice Manager, you will help create, refine, and implement the workflows, scope of practice, and processes that support patient care quality, efficiency, and clinical excellence across the organization. Key Responsibilities

Clinical Responsibilities

  • Provide timely, accurate, and empathetic clinical support across multiple communication channels, including patient portals, live chat, and community boards.
  • Perform clinical assessment through written communication (triage, symptom review, care escalation).
  • Manage and troubleshoot adverse effects, escalating appropriately to providers.
  • Interpret and track lab values; ensure follow-up and clinical recommendations are carried out.
  • Serve as a clinical escalation point for Care Coordinators and Providers.
  • Oversee patients off-boarding when care is no longer appropriate or safe.
  • Support high-risk and high-value patient needs, ensuring safe and compliant care delivery.
  • Participate in teaching—live or recorded sessions—to support patient learning and internal clinical education.

Administrative & Operational Responsibilities

  • Lead, coach, and continue to develop the Integrated Care team, fostering excellence, accountability, and continuous improvement.
  • Maintain SLA performance across teams; troubleshoot delays or workflow inefficiencies across CS/CC/RN.
  • Develop and refine clinical workflows, including but not limited to:
    • Care Coordination workflows
    • Lab and pharmacy workflows
    • Patient messaging flows
    • Off-boarding workflow
  • Partner with Product, Engineering, and Operations to improve tools and system functionality.
  • Collaborate closely with the Scheduling, Lab, and Messaging teams to ensure seamless execution of cross-functional processes.
  • Manage capacity planning and team allocation to match patient volume.
  • Serve as the primary representative of Nursing in platform improvements and operational changes.
  • Provide QA oversight for documentation, communication quality, safety processes, and protocol adherence.
  • Assist with the design and rollout of training materials for new hires (RNs, CCs).
  • Ensure alignment and clarity of roles across CS/CC/RN to streamline workflows and avoid redundancy.
 

Qualifications

Required

  • Current, unrestricted Compact RN license; CA licensure preferred.
  • 5+ years RN experience, including ≥2 years in telehealth or remote clinical care.
  • 2+ years clinical leadership or team management experience.
  • Deep understanding of digital health workflows and integrated care models.
  • Excellent written and verbal communication skills, especially in patient messaging.
  • Comfort with EHRs, patient communication platforms, and clinical decision support tools.
  • Ability to work cross-functionally and thrive in a fast-paced, evolving startup environment.

Preferred

  • Experience launching or scaling care coordination teams in a digital health environment.
  • Familiarity with hormone health, men’s health, chronic care management, or wellness protocols.
  • Background in workflow development, training material creation, or clinical operations.
