Deliver compassionate, high-quality nursing care as a Home Health Licensed Practical Nurse (LPN) supporting patients in their own homes. This full-time contract role is ideal for an experienced LPN who values patient-centered care and thrives working independently within a field-based setting. Strong clinical judgment and excellent communication with patients, families, and healthcare teams are essential.

Responsibilities:

  • Conduct skilled nursing visits to assess and implement personalized care plans
  • Administer medications, wound care, IV therapy, and other clinical interventions as ordered
  • Monitor patient conditions, promptly reporting changes to the RN Case Manager or physician
  • Educate patients and caregivers on disease management, medication, and self-care techniques
  • Accurately document all nursing care and communication using Home Care Home Base (HCHB)
  • Collaborate with interdisciplinary teams to ensure cohesive patient care and compliance
  • Maintain infection control standards and promote patient safety at all times
  • Participate in case conferences, quality improvement initiatives, and ongoing trainings

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum 1 year of home health, hospice, or community-based care experience
  • Proficient with Home Care Home Base (HCHB) electronic charting system
  • Valid drivers license, reliable vehicle, and current insurance
  • Current Basic Life Support (BLS) certification
  • Strong clinical judgment, time management, and communication skills
  • Ability to work independently and manage a dynamic caseload in a field environment

Preferred Skills:

  • Experience with Medicare-certified home health agencies
  • Knowledge of home health compliance and documentation standards
  • Patient-centered approach with strong interpersonal skills

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM, with some flexibility for patient needs
  • Approximately 35 patient visits per day (2532 points weekly)
