This role involves direct communication with patients, caregivers, and healthcare professionals to ensure accurate and timely delivery of medications.</p><p></p><p><b>Responsibilities</b></p><ul><li>Coordinate patient deliveries by making outbound and receiving inbound calls from patients, caregivers, and medical staff.</li><li>Conduct follow-up calls as directed by a licensed pharmacist.</li><li>Assist the specialty pharmacy team with tasks such as:<ul><li>Evaluating product usage</li><li>Managing and documenting supply inventory</li><li>Setting up medication orders</li><li>Performing verbal patient assessments</li></ul></li><li>Maintain and update patient records, medication profiles, and complete assessments under pharmacist supervision.</li><li>Accurately document all interactions in patient medical records.</li><li>Meet or exceed quality and productivity standards (e.g., 100–125 form setups daily).</li><li>Deliver exceptional customer service and maintain a professional, service-oriented approach in all interactions.</li><li>Act as a liaison between specialty pharmacy, operations, and patients, caregivers, or medical professionals.</li><li>Adhere to system-wide competencies and behaviors.</li><li>Perform additional duties as assigned.</li></ul><p></p><p><b>Requirements</b></p><ul><li>High school diploma or GED required.</li><li>Pharmacy Technician Certification preferred.</li><li>Prior experience in specialty healthcare preferred.</li><li>Proficiency in Microsoft Office Suite.</li></ul><p></p><p><span>As part of Evernorth Health Services, this role supports CarepathRx within </span><br><span>our Pharmacy and Care Delivery organization.
